    i was going to submit my review online, the paper that came with it said where to go but i lost the paper which is also my mail in review from, where do i go to submit it??

    Call member services, the number is at the bottom of the home page. MS does not do a good job of answering email. They will send you an email with a link to the test report form. Tom
