Yes I have, I was fishing from a rental boat at Morena Lake in San Diego county and I had just anchored to do some still fishing and as I bent over to get something out of the cooler she boat swung and I went head first into the water. I was unable to get back into the boat and so I grabbed the boat near the bow in my left hand and the anchor rope in my right and began scissor kicking (like sidestroke), towards shore. Another boat came along and helped get me in shallow enough that I could beach the boat and get to shore and dry out.
Poor Bill has been the water a bunch of time on film, and since it is on film, he can't deny any of it.

Pictured is a section of one of our favorite Steelhead rivers.  Water is ripping through here with several small falls known as Jack, Queen, King.  If you look close, you'll see me standing on the bank.  The canyon walls are steep and at times the river will cut you off from going any furher.  Just below where I'm standing, there is small ledge under the water that runs along the wall on the right.  As you work your way down stream, there is a hole in the ledge that is about another 2 feet deeper.  You are normally about knee deep and as you step down, you go about waist deep.  This was about 10 years ago, and I was working my way down the ledge, got to the step, stepped down, took another step and it went don again while I was expecting to step up.  Needless to say, I did a face plant, filled my waders, dropped the small plastic box full of corkies and jigs, but held my rod as I'm being washed downstream toward the waterfall at a pretty brisk pace. I managed to get my feet out in front of me and hit a rock which stood me up just before going into the chute.  I got to the side, made my way down to the boys and started fishing again.  After about an hour, it just got too cold to stay any more, so I told the boys I was headed to the truck.  When questioned, they apparently didn't know of my winter swimming party.  I don't go much lower than where I'm standing any longer, having survived my ordeal.

Yes to this. Once was standing on a very large tree limb casting out in a creek, shifted positions slightly, next thing I know gravity took over and... "Ker Splash" I was in the creek. Held on to my rod though and uttered a few unpleasant words as I walked / climbed up on the bank to dry out.

Last year Oct. 2011, a friend and I were fishing in a creek in a 12 ft semi-V bottom boat. We had fished in this boat a few times in the past with no trouble. My live bait hook was caught in a vine at the edge of the boat and I leaned over. At the same time my friend suddenly got up from his seat and I just rolled right out of that Darn boat, and he fell out as I nearly caused the boat to go over. Thank God the water was still warm. Amazingly no water got into the trolling motor or battery, and our tackle boxes stayed in the boat. I lost my bait rod for a few seconds. When I surfaced and saw my friend knee deep in water I looked for the vine I was hung in, and got my hook and pulled up my whole outfit. We emptied the boat of whatever water got in, with a large water bottle with the top cut off to dip water with.

We both laughed at the incident, got back in the boat and let the sun dry us out. I am sure we will remember this until the day we die. = LOL.
